Q. Do you do tubal ligation reversal? If so, how much does it cost? \n"); document.write("\n"); document.write("A. Yes, Dr. James Zolzer performs tubal reversals. This service is usually not covered by insurance plans. Our fee of $4500.00 must be paid in full prior to procedure. A. We typically schedule an initial appointment with one of our nurses/CMA's in order to begin your care. This appointment usually lasts 30 to 45 minutes and consists of a question and answer session and blood draw.
\n"); document.write("The first appointment with your physician is typically between 8 and 10 weeks gestation and usually includes an ultrasound along with the doctor visit.

Most insurance companies have an online provider directory and/or a member customer service phone number printed on your insurance card.
