Adolescent Health Screenings/Counseling

Though each young woman’s needs are different, the American College of Obstetrics and Gynecology suggests that adolescents should make an initial gynecological visit between the ages of 13 and 15. We are comfortable providing primary care for the female adolescent or can work in a collaborative role as consultant to the adolescent's primary health care physician. The initial visit does not necessarily need to involve a pelvic exam, but it can serve as an opportunity to meet and talk with one of our physicians in a question-and-answer session regarding adolescent development.

Our doctors will answer your questions about the HPV vaccine, which is prescribed for teens and young adults in the preventive treatment of the Human Papillomavirus, which can lead to some forms of cervical cancer.

Annual Examinations/Pap Smears

We recommend a yearly physical examination and pap smear as a regular component of preventive healthcare. A pap smear is a simple, quick and painless procedure that involves scraping cells from the surface of the cervix, preserving them in a fixative solution, and then evaluating the cells to detect those that may be pre-cancerous. Designed as a screening tool, the pap smear can indicate abnormal cell changes or the Human Papillomavirus (HPV), which can lead to cervical cancer if left untreated.

We also recommend the HPV test along with a pap smear for women ages 30 and above based on guidelines and regulations endorsed by the American College of Obstetricians and Gynecologists (ACOG), American Cancer Society (ACS) and American Society for Colposcopy and Cervical Pathology (ASCCP). Although HPV may resolve on its own, it may also return at any time – without symptoms – and the HPV test can help provide a more definitive indication of the presence of HPV than a pap test alone.

Cholesterol Screening

Many do not realize heart disease is the number one killer of women in the United States – ahead of breast cancer. One in 2.5 women will die of heart disease or stroke, compared with one in 30 from breast cancer. We recommend cholesterol screenings to detect elevated levels, which can lead to coronary diseases. Our physicians will discuss the need for a cholesterol screening and help suggest nutrition regimen and activities that can help keep cholesterol at a healthy levels.

Diabetes Screening

Nearly one-third of women (about 10 million) do not realize they have diabetes. Diabetes is the fifth deadliest disease in the United States and can be especially hard on women. Women with diabetes are also more likely to have a heart attack, and at a younger age, than women without diabetes. In pregnancy diabetes presents a unique situation, because the disease can affect both mothers and their unborn children. The disease can cause difficulties during pregnancy, such as a miscarriage or a baby born with birth defects.

For women who do not currently have diabetes, pregnancy brings the risk of gestational diabetes, which develops in about 2% to 5 percent of all pregnancies — but disappears when a pregnancy is over. Women who have had gestational diabetes, or have given birth to a baby weighting more than 9 pounds, are also at an increased risk for developing type 2 diabetes later in life.

Diabetes is at least 2 to 4 times higher among African American, Hispanic/Latino, American Indian, and Asian/Pacific Islander women than among white women. The risk for diabetes also increases with age.

All of our pregnant patients are tested and monitored for gestational diabetes. For those patients receiving gynecological and wellness services, our physicians will evaluate risk factors and discuss how to help prevent the disease. If a patient tests positive for diabetes, our physicians will work in tandem with an endocrinologist and/or a certified diabetes educator to provide proper treatment for the disease.

Endometriosis and Fibroids

Although endometriosis is a commonly diagnosed uterine disorder, it is often misdiagnosed or missed altogether. A common symptom is pain at the time of menstruation (dysmenorrhea) which occurs in almost all girls and women with endometriosis. Studies suggest that endometriosis is the cause of about 15 percent of cases of pain in the pelvic region in women.

Symptoms can vary widely, but our physicians are trained to determine whether pain is truly caused by endometriosis or some other condition such as pelvic inflammatory disease.

Uterine fibroids are tumors or growths, made up of muscle cells and other tissues that grow within the wall of the uterus. Most fibroids do not cause any symptoms, but some women with fibroids can experience a host of symptoms including heavy bleeding, pain during sex, lower back pain and others.

Although fibroids are sometimes called tumors, they are almost always benign (not cancerous). Fibroids can grow as a single growth or in clusters. Their size can vary from small, like an apple seed, (or less than one inch), to even larger than a grapefruit, or eight inches across or more. Drs. Female Incontinence

Incontinence is the involuntary loss of control of urine, usually resulting in leakage or wetting incidents. The common types that generally affect women include the following:

Stress incontinence - the loss of urine associated with anything that causes a woman to tense up her stomach muscles, such as coughing, sneezing, lifting, bending or exercise. This is the most common type of incontinence found in younger women and is typically related to trauma of the pelvic floor muscle structure from injury, surgery, radiation, but most commonly, childbirth.

Vaginal prolapse -  when the tissues of the pelvic floor that provide a support structure for internal organs, such as the bladder or uterus, become overstretched or weakened, these organs can prolapse or bulge into the vagina and stress incontinence may result. It is also related to childbirth and often accompanies stress incontinence.

Urge Incontinence - loss of urine for no apparent reason. Bladder contractions from abnormal nerve signals can be the cause of these spasms. Certain fluids, anxiety and some medical conditions, such as diabetes, can aggravate urge incontinence.

Overactive bladder - the need to urinate frequently and with urgency. Abnormal nerves send signals to the bladder at the wrong time, causing its muscles to squeeze without warning. This can cause disruption in daily routine and activities.
 
Many women believe incontinence is inevitable, but fortunately there is help for many of millions of women affected by this condition. Our physicians will provide the appropriate treatment for your type of incontinence including, strengthening exercises, medications and surgery when necessary.

Infertility Evaluation and Treatment

If you are under the age of 35 and you and your partner have been trying to conceive for one year without success; or if you are over the age of 35 and have been unsuccessful after trying to conceive for six months, it is time to seek medical treatment for infertility.

The first step in overcoming infertility is to complete an infertility evaluation. A number of tests that traditionally have been run as part of the initial work up but may be unnecessary in many cases. Some of these include diagnostic laparoscopy, post coital test and endometrial biopsy.

The results of the evaluation lead our physicians to determine the cause of infertility, which is most often related to age, ovulatory disorder, tubal factor or male factor.

Mammograms

A mammogram is used to aid in the diagnosis of breast diseases in women. It uses a low-dose x-ray system to examine the breast tissue and helps detect early breast cancer in women who have no other symptoms. It is also used to evaluate lumps, pain and other breast changes. For your convenience, the imaging center located in the same building as our office, typically offers same-day mammography screening.

Menstrual Abnormalities/Irregularities

Menstrual abnormalities include amenorrhea, dysfunctional uterine bleeding, dysmenorrhea (primary or secondary), and premenstrual syndrome. Irregular or absent menses and non-menstrual vaginal bleeding have many causes, but in women of reproductive age, pregnancy usually is suspected.

Obstetrics/High Risk/Multiples

The most immediate risk involved with multiple births is pre-term (or early) labor resulting in premature births. A typical, single pregnancy lasts about 40 weeks, but a twin pregnancy often lasts 35 to 37 weeks. Approximately one-half of all twins are born prematurely (before 37 weeks), and the risk of having a premature delivery increases with higher-order multiples.

Premature babies (preemies) can have numerous health challenges. Because the care of premature babies is so different from that of full-term infants, preemies are usually placed in a neonatal intensive care unit (NICU) after delivery. The risk of developing health problems increases with the degree of prematurity — babies born closer to their due date have a lower risk.

In addition to the possibility of premature births, other medical conditions that are more likely to occur during a multiple pregnancy include preeclampsia, gestational diabetes, placental problems, and fetal growth problems. Being part of a multiple birth can also be associated with long-term health problems in the infants. Developmental delays and cerebral palsy occur more commonly in twins than in single births, and there's a higher risk of enduring health problems with higher-order multiple births.

Because of these concerns, prospective parents may want to undergo counseling on the possibilities and risks associated with multiple births.

Osteoporosis

Osteoporosis is a disease that affects bone density and can lead to fragile bones. If left untreated, weakened bones can lead to dangerous fractures, typically in the hip, spine and wrist. Fractures of the hip and spine are especially serious, and almost always require hospitalization and major surgery.

Women are four times more likely than men to develop the disease. Certain risk factors increase your chances of developing the disease including, race (Asian, Caucasian women, and Latino women can be at higher risk), small-framed females, post-menopausal women, lifestyle choices, health, genetics and hormonal issues.

Risk factors can have an affect on whether or not a woman develops the disease or the severity of the condition. A simple screening procedure, however, can detect changes in the bone density, and medications can help slow the progression of the disease.

Our physicians will recommend the time to test for osteoporosis based on age and medical history and, upon diagnosis, will offer appropriate treatment options.

Pelvic Pain Diagnosis and Treatment

Pelvic pain can have many different causes. For example, pain can be caused by a gynecologic condition such as endometriosis, uterine fibroids, ovarian cysts or pelvic adhesions. It also can be the result of an intestinal, urinary or muscular problem. Pelvic pain can even be a manifestation of stress or depression.

Our physicians may conduct a medical history and physical exam or ultra sound examination to suggest whether or not the pelvic pain may indicate endometriosis. Determining a definitive diagnosis of endometriosis, however, requires surgical exploration, usually via laparoscopy.
